Lung diseases can develop for many different reasons. Some conditions are caused by infections, while others are associated with smoking, air pollution, occupational exposure, inherited genetic changes, immune-system disorders, or long-term exposure to substances that damage the respiratory system.

Understanding the causes and risk factors for lung disease can help patients recognize potentially harmful exposures, discuss their personal risk with a healthcare professional, and take appropriate steps to protect their respiratory health.

The factors involved depend on the specific condition. Asthma, chronic obstructive pulmonary disease, pulmonary fibrosis, pneumonia, bronchiectasis, occupational lung disease, pulmonary hypertension, cystic fibrosis, and lung cancer do not all develop in the same way. In many patients, several factors may contribute to the development or progression of disease.

Smoking and Tobacco Exposure

Tobacco smoking is one of the most important preventable risk factors for lung disease. Cigarette smoke contains chemicals and particles that can damage the airways, reduce lung function, destroy lung tissue, and contribute to chronic inflammation.

Smoking is strongly associated with chronic obstructive pulmonary disease and lung cancer. It may also worsen asthma, increase susceptibility to respiratory infections, delay recovery, and reduce the effectiveness of some treatments.

Second-hand smoke can also affect respiratory health. People who regularly breathe smoke from other people’s cigarettes, cigars, or pipes may experience airway irritation and have an increased risk of several respiratory conditions.

Stopping smoking can benefit respiratory health at any age. The degree of recovery depends on the condition and the amount of existing lung damage, but quitting can reduce continued exposure and may slow further deterioration.

Air Pollution and Indoor Air Quality

Both outdoor and indoor air pollution can affect the lungs. The World Health Organization identifies tobacco smoke, air pollution, allergens, and occupational hazards as major risk factors for chronic respiratory diseases.

Outdoor pollutants may include fine particles, traffic emissions, ozone, industrial pollution, smoke from wildfires, and chemicals released through fuel combustion. These pollutants can irritate the airways and may worsen conditions such as asthma and COPD.

Indoor air pollution may come from tobacco smoke, cooking or heating with solid fuels, mould, poor ventilation, cleaning products, chemicals, dust, or building materials. Household air quality may be particularly important for children, older adults, and people who already have a respiratory condition.

The health effects of pollution depend on the type of pollutant, concentration, length of exposure, and individual susceptibility.

Occupational Lung Disease

Some people are repeatedly exposed to dust, fibres, chemicals, fumes, vapours, or biological materials through their work. These exposures may contribute to occupational asthma, COPD, hypersensitivity pneumonitis, silicosis, asbestosis, lung cancer, or other respiratory conditions.

Potential workplace hazards may include:

Occupational lung disease may develop gradually, and symptoms may not appear until years after the exposure began. Patients with chronic cough or breathing difficulty should tell their healthcare provider about both current and previous occupations.

Protective equipment, workplace ventilation, exposure monitoring, and compliance with occupational safety standards may help reduce risk.

Respiratory Infections

Viruses, bacteria, fungi, and other microorganisms can cause infections involving the airways or lung tissue. Pneumonia, influenza, tuberculosis, COVID-19, and other respiratory infections may be temporary, but some can cause severe illness or lasting lung damage.

Repeated infections may also contribute to conditions such as bronchiectasis. People with weakened immune systems, chronic lung disease, advanced age, or certain medical conditions may be more vulnerable to complications.

Vaccination, hand hygiene, appropriate treatment, and avoiding close contact with people who have contagious respiratory infections may reduce some infection-related risks.

Genetic and Inherited Factors

Certain lung diseases are linked to inherited genetic changes. Cystic fibrosis is caused by variants in the CFTR gene, while alpha-1 antitrypsin deficiency may increase the risk of emphysema and liver disease.

Genetic factors may also influence how susceptible a person is to asthma, pulmonary fibrosis, pulmonary hypertension, or lung cancer. Having a family history does not necessarily mean that a person will develop the same disease, but it may justify closer monitoring or genetic evaluation.

Healthcare professionals may consider genetic testing when symptoms begin at an unusually young age, several family members have similar conditions, or the clinical findings suggest an inherited disorder.

Allergens and Immune-System Conditions

Allergens such as pollen, dust mites, animal dander, mould, and occupational substances may trigger asthma symptoms in susceptible individuals.

Some lung diseases develop when the immune system reacts abnormally. Autoimmune diseases such as rheumatoid arthritis, systemic sclerosis, lupus, and inflammatory muscle diseases may affect lung tissue or the blood vessels in the lungs.

Hypersensitivity pneumonitis can occur when the immune system reacts to repeatedly inhaled organic particles, such as mould, bird proteins, or agricultural dust.

Medications and Medical Treatments

Certain medications may cause inflammation, fluid accumulation, or scarring in the lungs. Radiation therapy involving the chest can also affect lung tissue in some patients.

The risk depends on the medication, dosage, duration of treatment, underlying health, and whether other therapies are being used at the same time.

Patients should not stop prescribed medication without medical advice. A healthcare professional may evaluate whether a medicine could be contributing to new respiratory symptoms and determine whether an alternative is appropriate.

Age and Early-Life Factors

Lung function changes naturally with age. Older adults may be more vulnerable to chronic respiratory disease because of accumulated exposure, reduced immune response, and other health conditions.

Events during infancy and childhood may also influence respiratory health later in life. Premature birth, impaired lung development, exposure to smoke, and frequent lower respiratory infections during childhood have been associated with an increased risk of chronic breathing problems.

WHO also identifies frequent childhood respiratory infections among the risk factors associated with chronic respiratory disease.

Reducing Lung Disease Risk

Not all lung diseases can be prevented. Genetic conditions, autoimmune disorders, and some forms of pulmonary fibrosis may occur even when a person follows a healthy lifestyle.

However, risk may be reduced by:

  • Avoiding smoking and second-hand smoke
  • Limiting exposure to polluted air
  • Using appropriate workplace protection
  • Improving indoor ventilation
  • Reducing exposure to mould and respiratory irritants
  • Following vaccination recommendations
  • Managing asthma and allergies appropriately
  • Seeking assessment for persistent respiratory symptoms
  • Discussing family history with a healthcare provider
  • Following safety instructions for medications and radiation treatment

Risk factors do not provide a diagnosis. A person may have several risk factors without developing lung disease, while another person may develop a condition without an identifiable cause.
